The Athletic Clearance Process

Getting an athlete cleared at Solar Urgent Care is a straightforward process. Here is how it works from start to finish.

Step 1 — Bring the right paperwork

Arrive with the physical form from your school or league, the health history section filled out, and any relevant immunization records. If you do not have the form, we have standard CIF forms available at the clinic.

Step 2 — Check in

Let the front desk know you are there for a sports physical. You will provide ID and insurance information or confirm self-pay. The health history form is reviewed before the exam begins.

Step 3 — The physical examination

A physician or physician assistant conducts the full examination covering all the components listed in the table above. The exam is sport-specific, so the physician will ask which sport or sports the athlete participates in.

Step 4 — Physician review and clearance

The physician reviews the findings with you and, if the athlete is cleared, signs and completes the physical form. You leave with a fully signed form ready to submit to the school or league.

Step 5 — If follow-up is needed

If anything comes up during the exam that requires further evaluation before clearance can be given, the physician will explain the concern, provide a referral if needed, and advise on the appropriate next steps. This is not common, but it does happen and it is an important part of the process.
